In Clinical Pilates and indeed any kind of management plan results speak volumes – and clinical Pilates has been shown to be highly successful in getting them.
So what exactly is it and how does it work?
Pilates itself is a method of exercise. It was devised by Joseph Pilates during WWII and was used to rehabilitate prisoners of war and later when he moved to America, to rehabilitate, re train and strengthen dancers. His studio was close to the New York Ballet and he grew in popularity as he was able to keep them moving, active and get them back dancing again quickly and more effectively.
Pilates is now main stream – you see it on DVD’s, studios have popped up everywhere, large gyms and sport centres offer it – but what is it really and what is Clinical Pilates and how does it work?
Pilates is a method of exercise that tries to ‘normalise’ movement. It gets you to move and think about what muscles you are using to do that movement with, and work into length and strength. It has been famed for getting a stronger core – getting long lean muscles and increasing flexibility, and it is true – it does all of these things.
BUT – you must understand HOW to move and MOVE WELL.
That can be in a group class for mat work – on the floor, or it can be in s studio set up where you can use the big specialist equipment such as the reformers, trapeze tables and wunda chairs or barrels.
So what’s the difference?
The original method was aimed at getting injured and sick out and able to walk and work on the floor. So it utilised springs, pulleys and straps to get them strong enough to work on the muscles lying down and then get into the upright position so they join in the ‘floor work’.
Therefore mat work is Pilates in its hardest form – you are supporting your own body weight and working against gravity.
Using the equipment allows us to isolate and support and strengthen so we can be a whole lot more specific with what we do and guide and strengthen with laser targeting.
Clinical Pilates takes that a level further.
If you have been told to do clinical Pilates – here’s what that means and what you should be doing.
We utilise the method of Pilates, it’s principles and the equipment and layer that in with our standard physio and rehab management to get you moving earlier in the acute stage, support you fully if you are in pain and scared about moving, or worried about causing further damage.
It means we can get you moving well in different formats, and strengthen and activate the correct muscle before you start trying to then LOAD those muscles.

Clinical Pilates is NOT:
- being given a set of exercises to just do in a room on your own – you should be monitored, corrected, adjusted at all times – do it well – or don’t do it at all
- blindly exercising without thought for from or technique – you should know what to move, when to move it – how it should feel, where you should work from and in what order.
- in my opinion – delivered by anyone non medically trained.
- exercise with intent – know what you’re working and why it’s important
- movement with purpose – each given thing should have an end goal – to stretch you, to mobilise something, to strengthen, or release
- manual techniques and assistance with movement via traditional physio hands on methods as required
- targeted and specific to the end goal
- understanding what your body is doing
- knowing what stage your body is in and why it is there and knowing what you need to do to move it to the next phase.
We have had so many clients say they have tried Pilates and it didn’t work, yet when we have spoken to them they have said they had their program to do on either a mat or reformer and they would attend 3 times a week – do their program and leave. No one watched them or corrected their form and they had no idea if they did it well or not – no wonder it didn’t work or they didn’t feel anything!
That is not Pilates – in any format – and that is NOT CLINICAL PILATES.
